All Paperwork DUE May 13th Please go to the Athletic Information section of this site for more information IMPORTANT INFORMATION If your child is having a physical : Ask the doctor to fax a copy to the health office at 229-2085. Or mail it to FDR school at the address listed above. If your child will be in 10th grade, wants working papers, or is interested in participating in sports, a current physical is required. If your child needs/wants medication in school (even Tylenol, Advil, Motrin, etc.) a doctor’s order AND parent permission are required. Please have your doctor fill out the medication administration form, available in the 'Health and Athletic Forms' section of this web site, and fax it to the health office. A new permission form is required each academic year. If your child has asthma and needs an inhaler, even occasionally, please have the doctor fill out the medication administration form (see above) and fax it to the school. We would be happy to keep an inhaler in the health office for your child. If your student has an allergy that requires Benadryl or an Epi Pen please have the doctor fill out the medication administration form (see above) and fax it to the health office. Also, please bring an Epi Pen to the health office for your child.
